This is a key fact that many people miss: the Calcasieu Parish Clerk of Court only issues birth and death certificates at its West Calcasieu Annex office in Sulphur, not at its Lake Charles location. The Sulphur office is at 1300 E. Napoleon Street, Sulphur, LA 70663. This office shares a building with the DMV. Phone: (337) 990-8279.
Hours at the Sulphur office are Monday through Friday, 8:00 AM to 4:00 PM. Certificate applications are not accepted after 3:30 PM, so arrive with time to spare. If you show up at 3:45 PM, they will not be able to process your request that day.
The fee for a death certificate is $26 per certified copy. Each additional copy costs the same amount. Death certificates are only available for deaths that occurred after July 9, 2012. For deaths before that date, you must contact Louisiana Vital Records in New Orleans. The Sulphur office cannot help with older records regardless of the circumstances.
Also worth knowing: death certificates are usually available about 30 days after the date of death. If a death just occurred, call ahead before making the trip to confirm the record is in the system. Requesting a certificate before it has been registered will result in a wasted trip. Who Can Request Lake Charles Death Records
Louisiana is a closed-record state under R.S. 40:41. Only certain people may request a certified death certificate. Death records are not public records in Louisiana, and the clerk will not issue one to just anyone who asks.
